Disabilities resulting from drug addiction or alcoholism are generally not eligible for Social Security Disability.   1. Physical evidence includes physicians reports and records; laboratory and diagnostic studies; physical or other therapy records; chiropractic records.
  2. Mental evidence includes psychiatric or psychologist reports; clinical social worker/therapist reports.
